AWARE Worldwide has changed. We have laid out a plan- a bold, ambitious, challenging strategy that will guide our work over the coming decade. While building on what is best about our organization, and leveraging our previous years’ experiences, we have developed a plan for change. This plan examines AWARE’s past successes, and explores the tools and resources needed for the upcoming years 2010-2020 to affect lasting change for the uninsured, underinsured, and Elderly, living with Disabilities/Chronic Illnesses/HIV/AIDS |

Create New Partnerships- Our best work combines the strengths, and resources of the public, private and non-governmental organization sectors.
Leverage of our expertise sets us apart as a leader in providing prevention education and services.
AWARE’s Programs:
- Medicaid Elderly and Disabled Consumer-Directed (EDCD) Services Waiver Program
- Personal Care/Respite Services
- Health Enhancement and Learning (H.E.A.L) H.E.A.L Forums to empower, and affect the well-being of caregivers of persons with disabilities, who may be living with Chronic Illnesses/Family Violence with information on prevention strategies, healthy lifestyle living, enhancement of nutritional habits for a better quality of life.
- Women’s Initiative for Safer Engagement (W.I.S.E) W.I.S.E. Women Groups will meet weekly to address issues surrounding chronic illnesses, mental health/substance abuse issues, domestic violence, employment/ financial resources, and social services assistance and referrals.
- E-Source Center provides one-to-one peer/health educator guidance and instruction navigating Health/Medical websites for resources and assistance with health management.
- Medication Assistance Program (MAP)
We will:
- Demonstrate Commitment-Climate change is creating new challenges, making it harder than ever for many people living with Chronic Illnesses/Disabilities/Family Violence to understand the importance of medication management, safer engagement, preventive care, and treatment due to lack of knowledge of resources, options, and consequences. AWARE will provide the needed tools to acquire such crucial information.
- Measure Impact- It’s not just about getting the job done; it’s learning what works, what doesn’t work, and why.
- Develop Innovative Funding Opportunities- AWARE speaks to the heads and hearts of our donors by offering proven programs that make a difference in the lives of all of the persons we serve who are living with, and affected by Chronic Illnesses/Disabilities/Family Violence.
- Build Our Global Component-AWARE Worldwide will offer strength, perspective and partnership in worldwide prevention efforts to stem Chronic Illnesses/Disabilities, and violence against womenand children.

Why change?Our vision demands it. As an organization committed to prevention education, and services for persons living with, or at risk of Chronic Illnesses/Disabilities, and Family Violence, we must do more than help uninsured, low or no income people meet basic needs of maintenance of illnesses and their challenges. That is a start, but not an end. We must do more than help individuals make incremental changes in their lives. That is important, but we are compelled to go deeper. We must implement innovations for resources and referrals, and knowledge bases for those living with and/or affected by these societal ills. AWARE’s new strategic plan is a major leap forward- a roadmap to deliver our vision in a way that meets all the aspirations behind it.
Why now? 1) The time is right for AWARE. We have developed a new leadership/management perspective that combines out-of-the-box thinking, and innovative thought processes with time-tested, field-based experience.
2) The time is right for the world. In January 2000, the Department of Health and Human Services launched Healthy People 2010, a comprehensive, nationwide health promotion and disease prevention agenda. Healthy People 2010 contains 467 objectives designed to serve as a framework for improving the health of all people in the United States during the first decade of the 21st century. Healthy People 2020 builds on the same initiatives pursued over the preceding decade with two overarching goals—to increase quality and years of healthy life, and to eliminate health disparities. AWARE Worldwide is on board with helping to achieve these objectives through its prevention programs, innovative thinking, and collaboration and partnerships efforts.
3) The time is right for our supporters. You recognize that we live in an interconnected society. What affects one of us, affects all of us. We are our sisters and brothers keepers. We know you are ready for change; ready to take on the big issues and challenges of the upcoming years, and ready to give a helping hand to your brethren, so that like yourself, he/she can live a safe and healthy quality of life. | |
